Recreation: % did not load

I'm running into an error when I use the recreation model (script below). Attached is the csv file I am using for the predictor table.

    Hi it looks like the "road_idn.shx" file is missing.  That ".shx" is part of a set of files that come along with ESRI Shapefiles.  Can you check that file to see if it is missing and/or if you can open your "road_ind.shp" file in a GIS desktop tool?

    Thank you for the quick response, unfortunately that doesn't seem to be the issue. I actually am able to open "road_idn.shp". I should also add that I'm receiving the same error no matter what file type I indicate on the predictor table csv file. Any other suggestions?
